Every Coach must have a current Police Check or signed Affidavit on file with Oakridge Hockey Coaches new to Oakridge must have a police check completed this
season.
Those coaches who have had a police check last season must sign an "Affidavit" indicating no Police involvement in the last 12 months.
Police Checks and Affidavits can only be completed "IN PERSON" at the assigned times.
2009 Police Checks Times and Dates:
Thursday October 22, 7:00 to 9:00pm Medway Arena
Saturday October 24, 8:00am to 10:00am Oakridge Arena
Tuesday October 27, 7:00 to 9:00pm Kinsmen Arena
Thursday October 29, 7:00pm to 9:00pm Medway Arena
IMPORTANT
Oakridge Hockey recognizes that a person with a minor criminal record may still be eligible to coach. However, any coach with a criminal record, no matter how dated, must complete a Criminal Record Check Annually. Any coach who's Police Record Check has shown any area of concern must complete a Police Record Check Completed Annually. Every Coach, Assistant Coach, On Ice Helper, Trainer and Manager must attend at one of the assigned time and complete a Police Record Check form or an Affidavit. Whichever is applicable.
Oakridge Hockey Reserves the right to have any person who is a Coach, Assistant Coach, On Ice Helper, Trainer or Manager submit to Police Record Check at any time while they are involved in activities with Oakridge Hockey.
This is the only form that can be accepted for this purpose. It can be filled out in adavance brought with you.
Affidavits must be completed "IN PERSON". Everyone must bring two pieces of Identification.
Frequently Asked Questions.
Q - I have a police check on file with the school. Can I use that check?
A - No. We do not accept copies from other organizations. That also applies to minor sport clubs.
Q - My child's school also wants me to get a check. Can I use the same one from Hockey?
A - No. Oakridge Hockey will pay for the check. Privacy laws only allow us to use that information for our purposes. Once the results of the check have been recorded, the hard copy is shredded.
Q - How long is the Police Check good for?
A - Hockey Canada requires us to have checks done very 4 years. However Oakridge Hockey requires coaching staff to sign an affidavit annually.
Q - I live out of town. Do I still do a London Police Check
A - No. If you live outside of the boundaries of London you must have your check done by the Ontario Provincial Police. You will have to do that at one of their detachments.
